Here we discuss about patient's problems with a series of inputs with an aim to solve them  A 67 YR M WITH SUDDEN DIFFICULTY IN SPEECH AND SWALLOWING CHIEF COMPLAINTS: • No speech since 3 days • Difficulty to swallow since 3 days • Not passing urine since 2 days HOPI: • Pt was apparently asymptomatic 25 yrs ago, when he developed HTn and DM and since then is on treatment for it.  • Later, 10 yrs ago, he developed SOB, pedal edema for which he was diagnosed with a heart problem and got opersted with 3 stunts.  • Again 4 yrs ago, he developed glaucoma in both of his eyes, for which he got operated for both eyes. Here we discuss about patient's problems with a series of inputs with an aim to solve them 60 YR OLD MAN UNABLE TO SPEAK CHIEF COMPLAINTS : • Increased abdominal pain since 10 days • SOB since 10 days • Unable to speak since 2 days HOPI: • Apparently pt was asymptomatic 6 mnths back • Then he developed puffiness of face, pedal edema, abdominal pain, back ache, fever, burning micturition for which he visited a local hospital.  • He got diagnosed with renal calculi and also shrunken kidneys for which he took medications and got relief.  • Since 10 days, he again developed increased abdominal pain and SOB • Also couldn't speak from 2 days PAST HISTORY: • k/c/o renal atrophy/shrunken kidneys since 6mnths • NSAID abuse since 20 yrs

Here we discuss about patient's problems with a series of inputs with an aim to solve them A 50 YR OLD FEMALE WITH ANURIA Chief complaints : Anuria since 3 days Fever with chills since 3 days SOB grade 3 since 3 days Throat pain with difficulty in swallowing since 1 week HOPI : Apparently pt was asymptomatic 1 week ago Then she developed few throat ulcers which made her difficult to swallow even water.  She got treated for it in a local hospital but it did not get cured.  Due to those ulcers, she couldn't even drink nor eat.  Since 3 days, she developed fever with chills, anuria, SOB Yesterday morning, they visited a govt hospital in suryapet, where they stayed till evening just to know that she was suffering with renal failur
