GM CASE DISCUSSION

Greetings to one and all who are currently reading my blog. This is Rithika, a third semester medical student. 
                     
                       This is an online e-log book to discuss our patient's de-identified health data shared after taking his/her/guardian's informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. 

A 60 YEAR OLD MALE WITH DENGUE

Chief complaints:

 Fever - 12 days back

History of present illness:

Patient was apparently asymptomatic 12 days back. 

• Then he developed 🤒fever with chills, which was continuously present throughout the day for 3-4 days. 

• Then , he was taken to a local private hospital, where he was diagnosed with low platelet count and was referred to a hospital in suryapet. 

• In suryapet, he was given anti-pyretics which worked only for a few hours and then again his fever came back. His platelet count didn't improve too. 

• Then he was referred to our hospital, and has been staying here since 3-4 days. 

• His platelet count has been improving, has no fever now and pt has reported that he's feeling much better now. 

History of past illness:

• Pt had no similar complaints in the past. 

• Has h/o DM, HTN since 6 months

• Also has h/o paralysis of right arm and slurring of speech since 3 months. ( Currently he's able to move his hand but slurring of speech still exists). 

Personal history:

He is married and an agricultural labourer by occupation

• He follows a mixed diet. 

• Has normal micturition and regular bowel habits. 

• Follows a normal sleep pattern, but has decreased appetite. 

• Has h/o alcohol intake and smoking chutta ( 5-6 times /day )daily 6 months back but discontinued it since he was diagnosed with DM and HTN

Family history:

• Has no significant family history

General Examination:

Has pallor
• No signs of icterys, cyanosis, lymphadenopathy, clubbing, swelling of fingers and toes, edema. 

Vitals:

• PR -
• BP -
• RR - 
• Temp -
• SpO2 -
• GRBS -
